New Patients

If you are joining our practice as a new patient, welcome! For your first visit, we will have a full examination of your entire mouth, teeth and TMJ. This will include X-rays, digital pictures and consulting your medical history. Below is a health questionnaire. Please complete both pages of the document and bring it to your first appointment.
